Dance, Animated! - Costume Payment

Please turn in this filled out form, with payment (cash or check), to the MS office in a sealed envelope by Thursday, April 30.  Please put the dancer’s  full name and grade on the envelope. Please know that we do our best to keep the cost as low as possible.
